Do I need a guided tour of the Vatican?

You need a timed ticket; the guide is optional and often worth it. The Vatican Museums are enormous and largely unlabelled in a way that makes sense, so a good guide turns a forced march to the Sistine Chapel into something you'll remember. Early-entry tours before general opening are the genuine luxury here — the Sistine Chapel nearly empty is a different experience from the Sistine Chapel with four thousand people in it. If you'd rather go alone, book the first slot of the day, walk briskly to the Sistine Chapel first, and work backwards against the crowd.
